To qualify for Home Health Care you must meet ALL of the following Criteria.

• Homebound Status
  • A patient is considered homebound if the patient is only able to leave his or her home occasionally and it is a “taxing effort” to do so. A patient may leave for brief periods of time such as doctor appointment, church, hair appointment, etc.
• Under the Care of a Physician
  • A Physician must order home health and establish a medical treatment plan with the home health agency. The plan of care must be reviewed by the physician at least every 60 days.
• Skilled Service Requirements
  • Skilled services must be medically reasonable and necessary to the treatment of the illness or injury. Services are short term and include occasional visits lasting between 30 minutes to an hour.
