Diabetes Education Programs


The ediba® Diabetes Center of Excellence team will help you make sense of the puzzle, with classes and personal consultations designed to meet your needs. Classes are led by certified diabetes educators
— nurses and dietitians — who help you learn how to manage your diabetes.

Follow these steps to get started:

  1. Talk to your doctor about which program is right for you. He or she can then refer you to the Diabetes Center for education based on your specific needs.
  2. Check with your insurance provider about your diabetes education benefits. Most insurance plans cover all or part of these programs.
  3. Call the number ofthe Diabetes Center listed on the back of this brochure for class time and information, or to schedule an appointmentfor class or individual consultation.


Individualized education and self-management training are offered in all aspects of diabetes self-care, including:

  • Diabetes disease process
  • Nutrition
  • Physical activity
  • Medications- pills and insulin
  • Monitoring
  • Preventing complications
  • Lifestyle changes
  • Diabetes and pregnancy
  • Insulin Pump Therapy

In addition, the ediba® Diabetes Center of Excellence offers a variety of specialized programs:

Program Outline

Comprehensive diabetes education/self-management training is offered in an ambulatory setting to adults and children (and their families) with Type 1, Type 2, and Gestational Diabetes. The education is provided individually or in group classes. Each individual participates in a needs assessment with an individualized education/self-management training plan developed by the patient – nurse / dietitian – physician team. Upon completion of initial education plan (usually consisting of 4 - 9 hours depending on patient’s needs and diagnosis) follow up is done through individual visits or phone contact. The participant’s progress in achieving education objectives, self-care behaviors, and outcome measures is evaluated at 3, 6, and, 12 months.
