Referral Process

The Anticoagulation Clinic Patient Referral Form must be completed and signed by the patient’s physician prior to a patient receiving a scheduled appointment. (Download form in Adobe .pdf)
 
Criteria for patient enrollment in the Anticoagulation Clinic:
  • Referring physician must have active privileges at INTEGRIS Baptist or Southwest Medical Center. 
  • Referring physician is providing ongoing medical care to the patient and
    maintains the collaborative practice relationship with the clinic.
  • Patient is ambulatory and able to come to the clinic for appointments. The clinic may provide short-term phone management for patients upon the discretion of the clinic. The clinic will not provide long-term phone management for patient on home health, hospice, or those getting venous punctures at other sites with INR results faxed to the clinic.
