Referral Process

The Anticoagulation Clinic Patient Referral Form must be completed and signed by the patient’s physician prior to a patient receiving a scheduled appointment. (Download form in Adobe .pdf)

Criteria for patient enrollment in the Anticoagulation Clinic:

Referring physician must have active privileges at INTEGRIS Baptist or Southwest Medical Center.
Referring physician is providing ongoing medical care to the patient and
maintains the collaborative practice relationship with the clinic.
Patient is ambulatory and able to come to the clinic for appointments. The clinic may provide short-term phone management for patients upon the discretion of the clinic. The clinic will not provide long-term phone management for patient on home health, hospice, or those getting venous punctures at other sites with INR results faxed to the clinic.
