The most important part of the screener's job is to respond to a referral and perform the screen in a quick and concise manner. It is also important to keep the referral source informed regarding the status of the admission.
Pre-Admission Process
Pre-admission staff participate in the referral intake. The appropriate screener is called to provide the patient information. The screener keeps track of the pre-admissions. A morning meeting is held to share a list of pre-admissions to determine the current status of each admission.
The screening process involves the following steps:
- Attend the morning meeting with the medical director;
- Set schedule of screens to be completed by order of priority, considering the date of discharge;
- Following the morning meeting, case managers/social workers for each case is contacted to provide rehab recommendation and room assignment. Next, the screener notifies the nurse assigned to the patient. The screener asks for any pertinent medical information as indicated to assess whether the patient is medically stable.
- Next, follow-ups from the screens initiated on the previous day are completed to ensure prompt response to case managers/social workers.
The patient and family, or significant others are oriented regarding the rehab process and what to expect. Special emphasis is given to the fact that a case manager/social worker will be assigned to their family to keep them informed of all future discharge planning.
Family members are encouraged to contact these resource personnel with any questions or issues. It is important to provide the family with a feeling of control and continuity in helping their loved ones.
A brochure is provided to the family that provides a copy of the patient information form and the business card of the screener. The screener offers tours readily to family members with a contact name and telephone number.
The screeners help set appointments for family tours. In some situations, the screener may opt to visit with family to set up the tour following their receipt of the PT/OT evaluation.
