INTEGRIS Mayes County Medical Center is committed to providing quality care to each patient. This is demonstrated by our Medical Staff's use of nationally recognized, evidence-based treatment protocols. To improve the care of heart failure patients, physicians and hospital staff have focused on discharge instructions. These instructions are very specific including activity, diet, weight monitoring, what to do if symptoms become worse, and medications.
INTEGRIS Mayes County Medical Center has developed a go home trifold format complete with plastic cover that can be carried in the patient'ss billfold or purse. This information form has a short medical history, vaccines, allergies, physicians and numbers and current medications/dosages.
We want this to be a community project, asking each physician's office to keep this current and are encouraged by the participation.
