At INTEGRIS Blackwell we are committed to providing safe care for our patients and quality care that is consistent with national guidelines. We look closely at how we are caring for our patients with heart failure. We have received national recognition for our care of CHF patients by receiving the 2007 VHA Clinical Excellence award.

We do very well at assessing how the heart is working by getting echocardiograms on our patients. When patients need medication to improve their heart function, we prescribe the appropriate medications. 

We have made steady improvement in our documentation of discharge instructions. It is important for patients to know how to care for themselves when they go home. We collect a list of the patient’s medicine when they enter the hospital and update the list when they are discharged. We tell the patient to monitor their daily weight, what they should eat and drink, what symptoms to watch for, and their activity level when they go home.  

We have improved documentation of smoking cessation education and have remained at 100% since October 2005. It is important to us to discuss the risks of smoking with our patients and encourage them to stop smoking.
